Discounted Care Opportunities at the Gaudiani Clinic

In an effort to expand expert medical care to people who would otherwise be unable to receive care at the Gaudiani Clinic, we offer several opportunities for discounted care:

General Eligibility Criteria for Aaron’s Gift Opportunities:

  • Open to individuals with current or past eating disorders/disordered eating

  • Open to individuals living in: AZ, CA, CO, CT, FL, GA, IA, ID, IL, IN, KS, KY, MD, ME, MI, MN, MS, MT, NC, ND, NH, NY, NV, OH, OK, RI, SD, TN, TX, UT, WA, WI, WY

  • Demonstrated financial need defined as approximately $80,000 or less per year

  • Individuals are able to afford the discounted out-of-pocket costs (i.e., $250 Expert Consultation fee, $250 membership fee, and $50 per 25 minute follow-up appointments fee). Recipients are also required to be able to obtain and pay for labs/other medical work ups as needed as well as maintain relationships with other clinical team members. Additionally, award recipients are required to pay for transportation and other costs associated with any in-person care in Denver (though we do not require in-person consultations).

  • Individuals have access to a commercial PPO/EPO insurance plan or have an in-network medical provider open to partnering with the Gaudiani Clinic if managed care is accessed through an HMO like Medicare or Kaiser. We are unfortunately unable to support individuals with an active Medicaid plan as a primary or secondary insurer.

  • Understanding and full consent that the discounted care lasts up to one year only,  Please check in with yourself before applying as we know this timeline for care may not suit all.  We are not able to offer extensions.

  • The completion of the discounted care application, which includes an intake form, Aaron’s Gift questionnaire, a provider letter of support, and requested financial information.  


Financial Information:

We're honored to offer need-based discounted care through our partnership with Aaron's Gift.  When granted, these scholarships cover nearly 90% of our clinic fees for up to one year.  Aaron’s Gift provides financial assistance up to the following amounts:

Expert Consultation: $250 patient contribution/$1,500-$2,750 Aaron’s Gift contribution

Membership Fee: $250 patient contribution/$1,500-$2,750 Aaron’s Gift contribution

Follow-up Visit Fees: $50 patient contribution/$125-$250 Aaron’s Gift contribution

All Aaron’s Gift contributions are paid directly to the provider.


Application Process:

1) Complete application which includes:
-Intake Form
-Aaron’s Gift Board Questionnaire
-Release of information for Aaron’s Gift
-Releases of information for current/recent care providers (e.g., therapist, dietitian, PCP)
-Financial needs statement
-Financial information including most recent W2 and most recent three paystubs
-Referral letter from a current provider.  Any provider can complete this.  It can be a brief document sharing some of your work together, what you’ve been up against, and how a scholarship could help.
2) Application review by the Gaudiani Clinic and Aaron’s Gift (can take up to four weeks)
3) Complete additional paperwork (e.g., consents and financial) if funding if awarded
4) Complete labs, imaging studies, etc. and schedule expert consultation if awarded

July 1-22, 2024
October 1-2, 2024

About Aaron’s Gift:

The Gaudiani Clinic is honored to be an approved provider for the Aaron’s Gift charitable foundation. Aaron’s Gift is a non-profit organization established to help families receive the best in specialized medical care for a loved with an eating disorder. It was established by the family of Aaron Michael Eyre in 2019. Aaron was born and raised in the Bay Area of Northern California. He battled a chronic eating disorder for over 18 years. Aaron had a brilliant mind, graduating from high school as valedictorian, then attending Brigham Young University, Ohlone College and the University of California Berkeley. He had a true fondness for the Chinese culture, was fluent in Mandarin, and spent a summer studying at National Taiwan Normal University in Taipei. He served as a missionary for the Church of Jesus Christ of Latter Day Saints in Toronto, Canada He has a deep love for basketball and was a loyal fan of the Lakers and later the Warriors. Aaron worked frequently as a tutor in math and science for high school and junior high school students. He was a gifted teacher and loved assisting students in learning principles that always came easily to him. His students and their parents were always full of gratitude for how patient, thoughtful, and helpful Aaron was.

All who knew Aaron were lifted by his kindness, his caring spirit, his brilliant smile, and the light that was always felt in his presence. He was deeply sensitive to the feelings of others. He was a true follower of Jesus Christ, always eager to serve others in whatever ways he could, often sacrificing his own needs to help them.

The Gaudiani Clinic and Aaron's Gift align in their mutual goal to provide access to expert medical care to patients who are struggling or who have struggled with an eating disorder or disordered eating.
